Sirona Parkinson's Service
This service is for people of all ages with a diagnosis of Parkinson’s. You must be registered with a Bristol, North Somerset or South Gloucestershire GP practice.
The Parkinson’s Service can provide information, advice and support for people with Parkinson’s and for the carers, families and healthcare professionals involved in their care. The service includes both Parkinson’s Nurse Specialists and Parkinson’s Specialist practitioners who carry out the same role.
Initial referral is via your Consultant, GP, or any other Healthcare Professional involved in your care.
Self-referral is encouraged once you are known to the service

How to book
Call the Patient Transport Hub at 0300 777 6688. You will need your NHS number, which you can get from your GP surgery or healthcare professional.
Eligibility
You are eligible if your Parkinson's prevents you from using other transportation or if you need medical equipment or the support of clinically trained staff during your journey.
Escorts
You can book an escort to travel with you, but they must be a recognized parent or guardian of an eligible patient under 16. Escorts are only allowed for those who genuinely need additional support.
